Mechanism of Action

Each component targets a specific type of skin invader:

  1. Itraconazole (1.0%)(Stewarz IT): The Antifungal It inhibits the synthesis of ergosterol, a vital component of fungal cell membranes. By creating “holes” in the membrane, it causes the fungal cell contents to leak out, effectively killing the fungus.

  2. Ofloxacin (0.75%)(Stewarz IT): The Antibacterial A fluoroquinolone antibiotic that stops bacteria from reproducing. It inhibits an enzyme called DNA gyrase, which is essential for bacterial DNA replication. Without it, the bacteria cannot multiply and die off.

  3. Ornidazole (2.0%)(Stewarz IT): The Anti-protozoal/Antibiotic While often used for internal infections, in a cream, it helps tackle anaerobic bacteria and certain parasites by damaging their DNA strands through the release of free radicals.

  4. Clobetasol Propionate (0.05%)(Stewarz IT): The Steroid This is a “super-potent” corticosteroid. It suppresses the release of inflammatory chemicals (like prostaglandins and leukotrienes). It reduces the redness, swelling, and intense itching associated with skin infections.

  • Broad Spectrum: It’s a “one-size-fits-all” solution. It covers almost every common cause of skin irritation (fungal, bacterial, and inflammatory).

  • Rapid Symptom Relief: Because of the Clobetasol (the steroid), the itching and redness usually disappear within hours, even if the underlying infection isn’t fully cured yet.

  • Convenience: Instead of applying three different creams, the patient only needs one.

Cons & Risks

  • Steroid Dependency (TSW): Because Clobetasol is extremely strong, using it for more than 2 weeks can lead to “Topical Steroid Withdrawal” or skin thinning (atrophy).

  • Masking Infections: The steroid can make a fungal infection look like it’s healing because the redness goes away, but it may actually be helping the fungus grow deeper into the skin by suppressing the local immune response (a condition called Tinea Incognito).

  • Resistance: Overusing combination creams contributes to “superbugs”—bacteria and fungi that no longer respond to standard medicines.

  • Side Effects: Prolonged use can cause stretch marks (striae), lightening of the skin (hypopigmentation), and unwanted hair growth.
